Please complete the below Prospective Staff Immunization form and Health Review Questionnaire in order to be cleared by the Occupational Health and Safety department to begin employment. Please call 416-461-8252 ext. 2802 for any questions about this documentation. These documents are to be submitted prior to 12:00 noon on the Wednesday before your first date of employment.
Please review and complete the appropriate document package below. If you have been hired as a part time, casual or contract employee please fill out the 'New Hire Package - Part-time, Casual and Contract' document. If you have been hired as a full time employee please fill out the 'New Hire Package - Full-time' document.
Each package contains an instructional onboarding checklist, the employment status and benefit option form, the provincial and federal tax forms, and the banking information form. It is important to have these documents in to the Human resources department (Room M.060) on or before your first day of employment. Along with these documents you will also need to bring with you:

You will only be required to return the quizzes and the signature pages from these packages. These documents will need to be completed within your first week of employment.
