Our program helps support you in managing Type 2 diabetes and pre-diabetes through self-management education, collaboration and community partnerships.

How does it work?

The Diabetes Education Team (nurse and dietitian) provides you with individual or group education, counseling and support related to your self-management or prevention of type 2 diabetes.  We offer programming for patients from the Bridgepoint Family Health Team and those from the community. Typically you first meet individually with the nurse and dietitian who help you determine your individualized care plan. 

The diabetes nurse educator can help you with:

  • Diabetes prevention
  • SMART goals designed to encourage physical activity and healthy coping strategies
  • Managing your blood glucose and cholesterol levels
  • Taking diabetes medications safely and effectively
  • Preventing and/or managing diabetes related complications

The diabetes dietitian educator can help you with:

  • Understanding how food and physical activity impacts blood glucose
  • Creating meal plans focused on your lifestyle
  • Weight, blood pressure and cholesterol management
  • Reading food labels, choosing foods and tips on eating away from home

How to access these services

  • If you are a patient of the Bridgepoint Family Health Team you can speak to your primary care provider and they will refer you to our program
  • If you are a patient of a different doctor or nurse practitioner you can have them refer you using this referral form
  • Or, you can always refer yourself to our program by calling us directly at 416-470-6690
